Job Description
Position Summary:
We are seeking a compassionate, detail-oriented Bilingual Care Coordinator fluent in English and Russian to join our team. The Care Coordinator will play a key role in supporting clients by facilitating access to services, improving communication between providers and families, and ensuring continuity of care. This position requires strong interpersonal skills, cultural sensitivity, and a deep commitment to client-centered support.
Key Responsibilities:
Serve as the primary point of contact for English and Russian-speaking clients and their families.
Coordinate services and care plans across interdisciplinary teams, including medical providers, social workers, and community resources.
Conduct client intakes, needs assessments, and follow-ups in both English and Russian
Maintain accurate and timely documentation of client interactions and service plans.
Advocate for clients by helping them navigate healthcare, social services, and other systems.
Schedule and confirm appointments, assist with referrals, and provide logistical support (e.g., transportation, interpretation).
Monitor client progress and address barriers to care or services.
Maintain compliance with confidentiality and HIPAA regulations.
Qualifications:
Fluency in both English and Russian (spoken and written) required.
Bachelor’s degree in Social Work, Public Health, Psychology, or a related field preferred; equivalent experience considered.
1–3 years of experience in care coordination, case management, or social services strongly preferred.
Strong interpersonal, organizational, and communication skills.
Culturally competent and sensitive to diverse populations.
Proficiency in using electronic health records (EHR) and case management software is a plus.
Ability to work independently, manage multiple tasks, and remain flexible in a fast-paced environment.
