Job Description

A Home Health RN, or Registered Nurse, is a healthcare professional who visits patients in their homes to provide medical care, including administering medications, monitoring vital signs, managing wounds, assessing health needs, and educating patients and their families on managing their conditions, all while collaborating with the patient's physician and other healthcare providers to develop and implement individualized care plans; requiring excellent communication and assessment skills to operate independently within the patient's home environment.

Home Health RN, Registered Nurse - Per-Diem 

  • Conduct comprehensive assessments of patients' health status, including physical, social, and psychological aspects.
  • Develop and implement personalized care plans in collaboration with patients and their families.
  • Administer medications and treatments as prescribed, maintaining accurate and detailed records.
  • Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
  • Educate patients and their families on health management, disease prevention, and safety protocols.
  • Coordinate with interdisciplinary teams to ensure cohesive and effective patient care.
  • Maintain compliance with all health regulations and standards of practice.

Qualifications

  • Current RN license in the state of California.
  • Minimum of one year of experience in home health.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver’s license.
  • Ability to work independently and manage time effectively.
