Family Guidance Centers, Inc seeks to support the behavioral health of individuals, families, and communities through our commitment to innovation, accountability, and compassionate care to facilitate positive, lasting change. ****$1,000 SIGN ON BONUS****1st $500.00: after 30 days of employment 2nd $500.00: after 6 months of employment Essential Duties: - Facilitate linkages/coordinate services with outside agencies for patients with unmet needs (i.e., childcare, transportation service, psychiatry, housing, etc.)
- Track referrals to identified recovery support services to monitor status of patients' needs.
- Deliver peer recovery person to person, either via telehealth or in-person, aimed at increasing recovery capital by empowering individuals through strength-based coaching, supporting linkages to community resources and the development of natural supports, supporting engagement in treatment services, and educating customers and their families about the conditions and process of recovery.
- Track attendance of newly enrolled patients to ensure they complete orientation within five days of admission, following up with those who missed orientation or medication days, or are otherwise lacking in engagement.
- Meet with patients regarding concerns, symptoms and refer to appropriate medical/clinical staff.
Work Schedule: Monday - Friday (9:00a - 5:30p)
Start Date: ASAP Work Location: 333 W. Evergreen St., Manteno, IL Pay Rate: Up to $18.00 per hour; based on experience Family Guidance Centers, Inc. is a Drug Free Environment (including THC and/or CBD).
DCFS clearance required.

Experience and Skills Education and/or Experience: High School Diploma / GED
required; minimum of one year of recovery from substance use, mental health, or co-occurring substance use and mental health challenges required.
Certificates, Licenses, Registrations: Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS) from the ICB required within one year of placement in position. A valid driver's license and insurance required.
