*Job Summary*
We are seeking a compassionate and dedicated Registered Nurse to join our healthcare team. The ideal candidate will possess a strong understanding of patient care standards and demonstrate expertise in various nursing practices. This role involves providing high-quality care to patients, ensuring adherence to medical protocols, and collaborating with healthcare professionals to deliver optimal patient outcomes. Looking for someone with a year plus of MDS experience.
*Duties*
* Conduct comprehensive assessments of patients’ health status and document findings accurately.
* Develop and implement individualized care plans based on patient needs and preferences.
* Administer medications and treatments as prescribed, while monitoring for effectiveness and potential side effects.
* Provide education and support to patients and their families regarding health management, treatment options, and preventive care.
* Collaborate with interdisciplinary teams to coordinate care, including case management and utilization management activities.
* Review documentation for compliance with standards, including ICD-10 coding and HEDIS measures.
* Participate in hospice care initiatives as needed, ensuring comfort and dignity for end-of-life patients.
* Utilize electronic health record systems such as Athenahealth for documentation and patient management.
*Qualifications*
* Current Registered Nurse (RN) license in the state of practice.
* Strong knowledge of anatomy, physiology, and nursing best practices.
* Experience in case management or managed care settings is preferred.
* Familiarity with documentation review processes and quality assurance standards.
* Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
* Ability to work collaboratively within a team environment while demonstrating strong leadership skills when necessary.
