Green Meadows Hospice is looking for an Admissions Nurse to join our team! 

An admissions hospice nurse is responsible for assessing patients' needs and coordinating their care as they enter hospice services. This role is crucial in ensuring that patients and their families receive appropriate support during end-of-life care.

Key Responsibilities:

• Patient Assessment: Conduct comprehensive evaluations of patients' medical, emotional, and social needs.
• Care Coordination: Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Education: Provide information to patients and families about hospice services, treatment options, and end-of-life care.
• Support Services: Offer emotional support and counseling to patients and families, addressing concerns and questions.
• Documentation: Maintain accurate and timely records of patient assessments, care plans, and progress notes.
• Compliance: Ensure adherence to regulatory standards and hospice policies.

Skills Required:

• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Compassion and empathy for patients and families facing end-of-life issues.
• Knowledge of hospice care principles and practices.

Work Environment:

• Typically works in patients' homes, hospice facilities, or healthcare settings.
• May require flexible hours to accommodate patient needs.

Rates:
Admission $350
Follow Up $75
Recert $90
