As a Wellness Nurse at Sunrise, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities

  • Identify ongoing needs and services of residents to promote the highest quality of care.
  • Participate in evaluations and documentation for residents with changes in health care services.
  • Assist the Resident Care Director (RCD) in conducting resident evaluations and data gathering.
  • Conduct monthly wellness visits for all residents.
  • Monitor and follow up with residents who have experienced falls or incidents.
  • Document all pertinent information in the resident wellness file.
  • Maintain communication with residents' families to ensure medical needs are met.
  • Contact attending physicians as necessary.
  • Ensure weights and vital signs are obtained monthly for each resident.
  • Maintain wellness files according to Sunrise policies and regulations.
  • Manage medical supplies and emergency kits for the community.
  • Provide clinical support to community team members as needed.
  • Understand and follow infection control practices.
  • Regularly monitor each resident's medication and treatment profile.
  • Participate in quality assurance and clinical meetings.

Qualifications

  • Graduate of an approved college/school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year of experience in home health, assisted living, hospital, or long-term care.
  • Knowledge of nursing practices and regulations related to resident care.
  • Strong assessment skills and ability to handle multiple priorities.
  • Effective written and verbal communication skills.
  • Competent in organizational and time management skills.
  • Proficient in computer skills, including Microsoft Office and Sunrise applications.
  • Willingness to work flexible hours, including weekends and evenings.
